Financial Information:

  1. Three years Business and Personal Federal Tax Returns (state returns are not necessary)
  2. Current Interim Business Financial Statement (less than 90-days old)
  3. Business Debt Schedule (PDF) balanced to current Business Financial Statement
  4. Aging of accounts receivable and payable (Excel) balanced to current Business Financial Statement
  5. Personal Financial Statement, SBA Form 413 (PDF)

Additional Items for Leasehold Improvements/Construction:

  • Completion of Environmental Risk Questionnaire (to be completed by the Seller of the Real Estate), if the purchase of real estate is part of transaction
  • Copy of Construction Breakdown
  • Copy of General Contractor's License
  • Copy of General Constractor's Resume
  • Plans and Permits
